Background Screening (CORI)

All volunteers and coaches are required to complete a CORI background check each season prior to your involvement in the hockey program.

CORI background checks are now being conducted via USA Hockey. You can log in to usahockey.com/backgroundscreen to submit the form for processing.

You must do this on a computer. The form will not work on a phone. 

• When prompted to enter a code, please use: 35615801.
• You will need to upload a picture of a valid photo ID to complete this process.

We will be informed of your eligibility determination once the background screen is completed. You do not need to send anything if you enter your valid USA Hockey number as requested on the forms.
